For new groups, please complete this form as accurately as possible. If the application is incomplete it will not be accepted until all fields are complete or corrected. After submitting your form, the application will be reviewed. If the application is correct and complete, you will receive a welcome email and will be listed on the Find a Group page on the website.
For groups updating their records, please make sure all fields are complete and accurate. Incomplete information may impact your group’s “Active” status. If your group’s information is blank, you may have submitted an incorrect group email address.
